Employee killed in trench cave-in

A sanitary sewer renovation project was progressing through a cut in a street. The trench had been sloped back at 1/2:1, except in an area near a storm sewer inlet, which did not allow for much sloping. No alternative means of protection was used. Employee #1 was grouting a 10 inch pipe into a manhole when the steep wall of type B soil collapsed, bringing a chunk of pavement down onto Employee #1. He was only been buried up to his waist in soil but sustained serious chest and head injuries and died within a few minutes.
