Investigation Summary
At 10:15 a.m. on April 7, 2016, Employee #1 was servicing or maintaining a forklift. He had removed the pins from the mast of the forklift. As Employee #1 was in front of the forklift, the mast fell forward and struck his head. He was pronounced dead at the scene.
