Employee killed when crushed between roller and hood

On May 22, 1999, Employee #1 used a stepladder to reach an area between the hood on a laminator and a free-wheeling roller. Coworkers speculated he was attempting to clean the fins inside the hood, which he had observed being performed previously. Employee #1 climbed between the hood and the roller. The hood descended and crushed Employee #1 between the roller and the hood at the waist level. Employee #1 was killed.
