Employee killed when crushed by rolling pipe

Employee #1 was operating a truck-mounted well drill at the base of an earthen levee. A 25 ft long by 36 in. diameter casing that was being placed atop the levee rolled down the embankment and struck Employee #1. He was crushed and killed.
