Employee killed in fall through roof

Employee #1 was removing sections of roof sheeting that had been temporarily set the previous evening to protect the insulation from rain. Employee #1 removed the screws from his end, picked up the sheeting, then stepped through the insulation. He fell 17 ft to the concrete and landed on his head. He was killed. No fall protection was provided.
