Employee's leg amputated by air freight pallet

Employee #1 and coworkers went out onto a roller bed trailer to walk a 4-ton air freight pallet onto the dock. Employee #1 fell between the dock and the trailer and his leg was amputated by the rolling pallet. The space between the trailer and the dock was left unprotected, and proper procedures were not followed.
