Employee's finger amputated on table saw

On April 4, 1998, Employee #1 was using a table saw to cut plastic stock. The plastic piece became caught by the saw blade and kicked back at him. Employee #1's right hand struck against the blade, amputating his middle finger to the first knuckle and severely lacerating his ring finger. The table saw was not guarded.
