Employee dies after fall from stepladder

Employee #1 was securing a rain gutter downspout to the wall of a structure when he fell from third step of 6 ft stepladder. His head struck tile that had been installed over a concrete surface. Employee #1 suffered a massive head injury, from which he subsequently died.
