Employee fractures heels after falling and landing on feet

At approximately 2:05 p.m. on May 13, 1994, Employee #1 was approximately 15 feet high on a ladder pointing off the back side of a block wall. The ladder was set up on a portion of a finished concrete floor to which sealant was applied. The surface was very slippery. Shortly after Employee #1 climbed the ladder, it slipped from underneath him, causing him to fall to the floor. He landed on his feet and suffered severe fractures to both heels. He was hospitalized.
