Employee killed in fall from pallet elevated by forklift

Employee # 1, a maintenance worker, was repairing an overhead fluorescent light fixture. He was standing with a coworker on a wooden pallet that was elevated 12 to 15 feet above a warehouse floor by a forklift. For some reason, Employee #1lipped and fell off the pallet, striking the concrete floor. He suffered fatal head and internal injuries.
