Investigation Summary
Employee #1 had finished assisting two carpenters on the deck of a garage that was under construction. While stepping onto the ladder to descend, Employee #1 apparently kicked the ladder out to the right. Employee #1 fell over 11 ft to the frozen ground, sustaining fatal head and internal injuries. The ladder was not properly extended above the landing, and was not secured in place. The employees lacked proper safety training.
