At approximately 9:00 p.m. on February 21, 2016, Employee #1 was building a store product display when he was found near a ladder on the ground. Position of Employee #1 was not consistent with a fall from a ladder. Employee #1 was hospitalized with a head contusion and later died from a blood clot.
