Investigation Summary
At approximately 2:15 p.m. on April 30, 1993, Employee #1 was preparing to cut a 2 in. by 24 in. by 36 in. long block of styrofoam into 2 in. by 2 in. by 36 in. long strips. The table saw's guard and kickback device had been removed before the project was started. While cutting the material, Employee #1's right hand slid on the surface and came in contact with the saw blade. Two of his fingers were amputated and his thumb and index finger were severely lacerated.
