Employee killed in fall through floor hole

Employee #1 was standing in the kitchen area of a residence under construction, handing plywood up to a coworker on the roof. A floor hole measuring 36 1/2 in. by 114 in. had been cut the previous day. The hole was partially protected by 2 by 4 studs that were spaced 16 in. apart on their centers. Employee #1 stepped backward between the 2 by 4 studs and fell 9 ft through the floor hole. He was killed when he struck his head on the concrete basement slab.
