Employee killed when lacerated by edger

Employee #1, the operator, was at the rear of an edger applying oil to the rollers, shaft, and saw blades. A coworker activated the machine and Employee #1's arm and head were severely lacerated. It appears that he died instantly. The edger had been turned off, but it was not locked or tagged out
