Investigation Summary
Employee #1 was in a trench that was 3 ft wide at the bottom by 16 ft wide at the top by 16 ft deep and 20 ft long. He was waiting for pipe to be laid. The side of the trench caved in and completely covered the employee. Employee #1 was able to uncover his head. A rescue squad dug out the employee. He was hospitalized.
