Investigation Summary
Employees of D. Powell Enterprises were installing 42 in. corrugated metal pipe as an overflow from an impoundment at an abandoned strip mine. The trench was approximately 40 ft long by 9 ft wide by 5 ft deep. Employee #1 was kneeling and placing rocks under the pipe to level it. Employees #2 and #3 were holding up the pipe for Employee #1. The side of the trench caved in and buried Employee #1 up to his shoulders. Employees #2 and #3 were buried up to their hips. Employee #1 died of compression asphyxia. Employee #2 suffered back injuries and a fractured thumb. Employee #3 suffered bruises and a strained knee.
