Investigation Summary
At 10:30 a.m. on February 3, 2016, an employee was working near a Hobart dough mixer. It is unknown how the employee was pulled into the mixer. She was reportedly wearing a work uniform which consisted of white slacks, white polo shirt, and a thin white apron. The employee may have been wearing a long sleeve shirt under the polo. he employee was pulled into a Hobart V-1401 mixer, serial number 11-1005-003, 5 hp. The mixer is rated at 1725 rpm. No guards were in place and there was no guard for it on the premises. The employee was killed after sustaining blunt and sharp force trauma to the head, neck, and arm.
