EMPLOYEE WAS HIT BY A FORK LIFT

At about 10 a.m. on April 24, 1994, Employee #1 of Constar International was seriously injured. He suffered a broken right leg after he was hit by a forklift inside the carton forming area of the plant. Before he was hit by the forklift, Employee #1 was removing a plastic bottle that got struck in the overhead. He was admitted to a local hospital and was released the next day to go home to recuperate. Employee #2 was operating the Hyster forklift. He apparently did not look back. Employee #1 was later interviewed on the phone about the accident.
