Investigation Summary
At approximately 12:15 p.m. on June 1, 1995, Employee #1, of Best Delivery Service, was delivering two steel grates (catwalks) that measured 3 ft by 20 ft and weighed 1,092 lb. As he untied the grates from the wall of the trailer truck, the metal grates fell onto his right foot at the same time he was holding the grates with his hands. His right ring finger became caught between the sharp edges of the grates. Suffering a finger amputation, Employee #1 required hospitalization. Best Delivery Service was cited for the serious violation of T8CCR 3384(a) and T8CCR 3203(a)(7). Other violations were found and the employer was cited accordingly.
