Investigation Summary
At about 2:30 p.m. on May 24, 1995, Employee #1 and a coworker, of The Horizon Company, were working on a single-family home in Pasadena, CA. They were putting plywood underneath the house in order to retrofit the basement for earthquakes. A piece of wood had become stuck in the guard of the Skil saw during cutting and the guard did not retract when the saw kicked back. Employee #1 suffered a partial amputation of his left index finger and thumb. No direct safety violations could be established based on Employee #1's statement; however, a regulatory violation was cited for not notifying Cal/OSHA about the accident in time.
