Investigation Summary
At 2:30 p.m. on May 25, 1994, Employee #1, who was alone and performing his assigned job, was removing cargo from a truck deck and stumbled over wooden strips and metal bindings that had been cut prior to unloading the truck. He fell to the floor of the truck and injured his neck and left shoulder. He was taken to Brea Community Hospital and kept for overnight observation.
