Investigation Summary
Employee #1 was at a height of 17 ft 6 in. on a metal tubular scaffold, attaching drywall to the exterior of a building under construction. Lateral forces caused the scaffold to tip. Employee #1 fell between the scaffold and the wall and crashed against a staircase support, at which point he was knocked unconscious. He then fell onto a concrete slab at ground level. The scaffold was not tied-off or otherwise secured.
