Investigation Summary
At 1:40 p.m. on May 14, 1991, Employee #1 suffered a minor head injury when the 3-ton Milwaukee chain pull dislodged from the Caterpillar forklift. He was treated on site and never left the workplace. The employer and Employee #1 agreed that the safety latch on the chain pull hook had not been fully closed, letting the chain pull dislodge under pressure.
