Employee's hands burned by erupting hot water

At 4:00 p.m. on August 27, 1991, Employee #1, of George Blair Engineering, was adding soap to a heat treatment tank. Too much soap was added, forcing hot water to erupt and burn Employee #1's hands. Employee said that he had never witnessed a management inspection and he was never given formal training by management. The employer admitted to not having a complete I.P.P. program.
