Investigation Summary
Employee #1 was working 11 1/2 ft high on a 6:12 pitched roof. The rolled roofing material was left extended 5 inches over the edge because of the installation of other construction material later. Employee #1 knew of the 5 in. of unsupported roofing material but forgot and stepped on the edge. He lost his balance and fell, landing on his face and breaking a cheek bone. He could possibly lose one eye. No citation was issued for this accident.
