Investigation Summary
Employee #1 was helping to lower an approximately 20 ft long by 6 ft wide harrow off the wheels. The wheels are used to pull it down, and there was a bolt that held the other wheel on the back side when the harrow was being pulled in the field. When the nut was taken off this bolt, the harrow dropped and an approximately 3/4 in. diameter tooth went through Employee #1's foot. The harrow had been built about 10 years earlier by Barke Blacksmith Shop. One wheel was built so that when the nut was loosened, the tap pressed up against the harrow shaft. The opposite wheel, which did not press against the harrow shaft, let the harrow drop. The operator indicated that this was the first time during his 10 years of experience that the harrow had dropped.
