Investigation Summary
Employee #1, a plant maintenance mechanic, had climbed a forklift to position himself between the mast and a structure when his foot activated the mast. It moved backward, crushing Employee #1. The forklift operator shut off the unit, then turned it back on to tilt the mast forward and release him. Employee #1 sustained crushing injures that required hospitalization.
