Employee injured by fall when scissor lift overturns

Employee #1, of Borbon, Inc., was painting from a 25 ft scissor lift when he accidentally ran it into an approximately 6 in. deep hole. The lift tipped over and he rode it down as it fell. Employee #1suffered a fractured arm, for which he was hospitalized. He later said that he had been trained on lifts, and did not know why he didn't check the area before moving the lift forward. The lift company indicated that no mechanical deficiencies had been found when the lift was in the shop for routine maintenance.
