Investigation Summary
At about 6:30 a.m. on November 3, 1990, Employee #1, a machine operator at Mask-Off Company, Inc., started work in the unwind coating area. There was some dirt on the roller, so Employee #1 told a coworker to slow down the machine so he could quickly clean the roller. He went into the machine area and waited for the roller to slow down. When it did not slow down, he pivoted on his right foot to turn around and speak to the coworker again. Employee #1's left foot slipped forward and he lost his balance. As he fell, he extended his right hand and it became caught by the in-running roller. His right upper arm and elbow were crushed before the coworker could stopped the machine. The supervisor was called and Employee #1 was taken to the hospital.
