EMPLOYEE FALLS FROM UNSECURED LADDER

AT ABOUT 1 P.M. ON NOVEMBER 27, 1990, EMPLOYEE #1 FRACTURED HIS LEFT ANKLE WHEN THE WOODEN LADDER HE WAS STANDING ON ACCIDENTALLY SLIPPED, CAUSING HIM TO FALL ABOUT 25 FEET TO THE CEMENT FLOOR. EMPLOYEE #1 WAS UP ON THE LADDER NOTCHING THE CEILING WOOD PRIOR TO PIPE PLACEMENT WHEN THE LADDER SLID. THE MAKESHIFT LADDER, WHICH WAS ABOUT 20 FEET LONG, WAS RESTING AGAINST THE WALL ON THE NORTHWEST CORNER OF BUILDING #1 AND WAS NOT SECURED OR TIED AGAINST DISPLACEMENT. THERE WERE NO SAFETY PADS UNDERNEATH. EMPLOYEE #1 WAS TAKEN TO POMONA VALLEY MEDICAL CENTER BY PARAMEDICS FOR TREATMENT. HE WAS HOSPITALIZED FOR A FEW DAYS AND RELEASED TO RECUPERATE AT HOME.
