Employee's leg crushed between forklift tire and fender

At approximately 9:49 a.m. on March 12, 1993, Employee #1, age 18, of Kumho Tire, was attempting to ride on the forks of a forklift as it was backing up. His left leg became caught between the forklift's tire and fender. Employee #1's left ankle and leg were severely crushed.
