Investigation Summary
At approximately 2:00 a.m. on August 31, 1990, Employee #1 was loading a four-wheeled cart at the computer operations building. When he arrived at the shipping and receiving building to unload the boxes, his supervisor noticed that he seemed disoriented and that his head was bleeding. The supervisor asked what had happened and Employee #1 responded that he did not remember. Employee #1 apparently slipped and fell, lacerating his head.
