EMPLOYEE INJURED IN FALL THROUGH UNGUARDED ROOF OPENING

AT ABOUT 3 PM ON DECEMBER 17, 1990, EMPLOYEE #1 WENT ONTO THE ROOF OF A STRUCTURE TO REMOVE A FIRE HOSE THAT WAS USED TO TEST THE STRUCTURE'S DOWNSPOUTS AND PLUMBING. IN THE PROCESS OF STRAIGHTENING AND DRAINING THE REMAINING WATER FROM THE FIRE HOSE IN PREPARATION FOR ROLLING IT UP, EMPLOYEE #1 ACCIDENTALLY BACKED UP TOWARD A ROOF OPENING AND FELL FROM A HEIGHT OF 21 FEET 6 INCHES. HE FIRST HIT HIS STOMACH ON THE METAL RAIL OF A SCAFFOLD BEFORE HE LANDED ON THE CONCRETE GROUND FLOOR. EMPLOYEE #1 WAS BROUGHT TO HUNTINGTON MEMORIAL HOSPITAL IN PASADENA WHERE HE WAS TREATED FOR A BROKEN RIGHT ELBOW, WRIST, AND THUMB. ALSO, HE UNDERWENT SURGERY FOR A STOMACH PUNCTURE. HE STAYED IN THE HOSPITAL FOR ALMOST 2 WEEKS. THE MAIN CAUSE OF THE ACCIDENT WAS THE UNGUARDED ROOF OPENING AND THE FAILURE TO USE A SAFETY LINE OR A LANYARD IN THE ABSENCE OF A GUARDRAIL.
