Investigation Summary
At approximately 11:00 p.m. on May 7, 1992, at Checks-In-The-Mail in Irwindale, CA, Employee #1 was being trained in the printing of personal and business checks. On his first day, Employee #1 was oriented in the operation of the Heath turret check press machine. On the second day, May 7, he was first trained in changing the check plate for the numbering operation of the machine. The operator/trainer told Employee #1 to go to the back of the machine as he (the operator) worked on the check plate. Employee #1 had previously allegedly observed the operator retrieve paper scraps from the delivery area, making him believe it was a normal work procedure. When he saw a piece of paper scrap, Employee #1 opened the cover and reached for the paper scrap in the delivery area. Without warning, the operator turned on the machine and Employee #1's right arm was caught and fractured by the running delivery bar. He was brought to Queen of the Valley Hospital in West Covina, where he had surgery on his arm.
