Investigation Summary
At about 12:00 noon on November 12, 1992, Employee #1, of Marvin G. Tovey Construction, Inc., was working on a partially-tiered section of a roof on a building in City of Industry, CA. Roof renovation was in progress and the contractor had about eight employees working under the supervision of a job foreman. Employee #1 was crossing the roof's deck, approximately 4 feet by 8 feet, to collect hand tools when two 4 foot by 8 foot by 3/8 inch plywood sheet coverings and support beams/joists underneath gave way, causing him to fall about 17 to 25 feet. The section of roof that collapsed appeared to suffer from dry rot. Employee #1 stated that he accidentally stepped over a hole with joists underneath. He suffered fractured right and left hands and injuries to his right knee and forehead. Employee #1 was hospitalized at Harbor Medical Center in Carson. The broken coverings and wood joists were not available at the site on inspection day.
