Investigation Summary
At approximately 9:20 a.m. on June 14, 1991, Employee #1 was using a vacuum cleaner to clean the top of a Borctor Swartz pet food dryer, serial #K23324, when he slipped and fell on a motor. His right hand became caught between the unguarded V-belt and the pulley, and he sustained an open fracture and dislocation of his right third finger. He was transported to Queen of the Valley Hospital for treatment.
