Investigation Summary
At approximately 11:40 a.m. on May 2, 1997, two employees were using a forklift to move cement-cast cornices. Employee #1 was standing on the lifting forks to help secure the load during transit. The load shifted, causing Employee #1 and the load to fall from the vehicle. Employee #1 was transported to Henry Mayo Hospital in Valencia, where he was hospitalized for 21 days for treatment of head injuries.
