EMPLOYEE #1 WAS STANDING ON THE LIFT OF A FORKLIFT TRUCK USING A PALLET AS A PLATFORM. AS THE LIFT WAS LOWERED, HE HELD ON TO THE FORKLIFT CARRIAGE FOR SUPPORT. THE OPERATOR RETRACTED THE FORKS AND AS HE LOWERED THEM, HE CAUGHT THE EMPLOYEE #1'S HAND BETWEEN THE CARRIAGE AND BOOM LIFT. EMPLOYEE #1 RECEIVED SEVERAL CRUSHED FINGERS.
