Investigation Summary
At 4:30 a.m. on November 30, 1996, Employee #1 was injured when she fell from a 12-foot metal ladder. Employee #1 had climbed the ladder to reach a box of stuffed animals from the "overstock" shelf at approximately 8 feet in height. When Employee #1 lifted the box with some force anticipating a heavier weight, she was thrown off balance and fell about 8 feet to the floor. Employee #1 received multiple injuries including a fractured right wrist, fractured right shoulder, and a head contusion. Employee #1 was treated at the West Hills Hospital and released on December 1, 1996.
