Investigation Summary
At approximately 12:50 p.m. on September 20, 1996, Employees #1 and #2 of MCM Construction, Inc., were on a shoring support beam about 27 ft above hard-packed earth. They were installing shoring for the construction of a highway bridge. The crane operator swung a sling and spreader bar assembly toward them. As he reached for the assembly, Employee #1 lost his balance; he then reached back and grabbed Employee #2, causing both of them to fall. Employee #1 fell to the ground; he was wearing a full body harness, but the shock-absorbing lanyard was not attached to an anchor point. Employee #2, who was wearing a safety belt with a 12 ft long by 3/8 in. diameter wire rope lanyard, fell 10 ft. Paramedics were called and both employees were transported to Henry Mayo Newhall Memorial Hospital. Employee #2 was released at 4:30 p.m.; Employee #1 was hospitalized for treatment of multiple fractures.
