Employee crushes finger in dough machine roller

TAD Technical Services provided Employee #1 to Chef America, Chatsworth, which had direction and control of the employee. Employee #1 was trained in the safe procedures for cleaning the dough rolling machine; however, he did not turn the conveyor off while wiping down the conveyor belt. His glove was caught by the roller, crushing his right index and middle fingers. That middle finger was amputated to the first joint. No accident related citation was issued.
