Employee's foot is run over by forklift

Employee #1, who was in a forklift operations area, was not seen by the forklift operator as he came around a corner. The wheel on the forklift struck the side of his left foot, causing a severe laceration. No accident related violation was issued. The forklift operator was disciplined for not observing adequate caution when going around a corner.
