Investigation Summary
At approximately 10:15 a.m. on April 21, 1993, Employee #1 was elevated on a model 975 O'Connor Ladder Co. electric lift to a height of approximately 14 ft, painting a 20 ft high by 60 ft wide billboard canvas. He had been painting for about three hours when the lift platform suddenly fell to the floor. He was thrown to the floor of the platform and suffered multiple contusions and strains to his head, neck, lower back, right arm, and leg. Employee #1 was treated at the hospital and released the same day. He returned to work June 11, 1993.
