Investigation Summary
At approximately 2:15 p.m. on February 10, 1997, Employee #1, of Steg Manufacturing, was helping to place steel plates to cover a utility trench. The plates were being moved by a rough terrain forklift. Employee #1 was stabilizing a 224 lb plate when he was pushed backward. The forklift's right front tire caught his heel and his right foot was run over. He was transported by the Los Angeles Fire Department to Holy Cross Hospital, where he was treated for crushing injuries.
