Investigation Summary
At about 11:30 a.m. on December 8, 1993, Employee #1 of G&G Fire Sprinklers, Inc., was struck by an elbow pipe at Lockheed Aerospace in Palmdale, CA, in building 637A. The general contractor constructing building 637A for Lockheed Aerospace subcontracted the installation of the fire sprinkler system to G&G Fire Sprinklers, Inc. Employee #1 and a coworker were in the process of repairing or realigning the pipes to relocate the hose cabinet box. Employee #1 and the coworker were located in the northeast ceiling wall area on a lift at the time of the accident. They were working on a pressurized (220 psi) fire sprinkler system while the system was being tested. Employee #1 suffered serious injury to his voice box and respiratory system and was taken to Antelope Valley Hospital in Lancaster. Employee #1 did not receive training in hazard recognition, signs or tags were not posted to indicate that the fire sprinkler system was under pressure and being tested, and no energy control procedures had been developed.
