Employee's foot crushed by falling lift forks

At approximately 9:30 a.m. on December 8, 1993, Employee #1 was in the process of repairing a Hyster, an 8,000 pound capacity forklift. Employee #1 had placed a 12 inch high by 18 inches by 10 inch wide block consisting of two by fours nailed together in between the forks. The forks were stuck in the up position about 48 inches off the ground, and the carriage was not resting on the block. Employee #1 had been standing on the block while rethreading bolt holes on top of the cylinder. After a 20 minute break, Employee #1 returned to the forklift and walked up between the forks to step up on the block. As he approached the block, the forks suddenly dropped, hit the block, and landed on his left foot. Employee #1's injuries resulted in an initial hospitalization of 3 days for surgery on his left little toe, and in a second hospitalization on December 30 for surgical amputation of the little toe.
