Investigation Summary
On May 11, 1998, Employee #1, a mechanic, was hit in the chin and above the eyes by the hydraulic lines while inspecting a forklift that had broken down at the recycling facility of Santa Clarita Recycling. The mast had become stuck in the "up" position at the top of the ram. He tried to release the hydraulics using the controls, but the ram would not move. He stood on the fender of the machine to look at the ram, and the hydraulic lines of the fork assembly suddenly dropped. The hydraulic lines produced a cut on his chin and above his eyes. He was taken to Henry Mayo Hospital by paramedics and hospitalized. The doctors inserted stitches for the lacerations on his chin and above his eyes.
