Investigation Summary
Employee #1 was one of two employees working on a platform mounted on one side of a scissor lift platform to reach an inaccessible area. The employees were instructed to install a railing on the perimeter of the platform prior to use but failed to do so. They were elevated approximately 12 ft and had completed their work. Employee #1 backed up on the platform, without looking, to move the controls to come down, and fell. He suffered a left wrist fracture and a cracked rib.
