Investigation Summary
At approximately 3:20 p.m. on February 11, 1991, Employee #1 and coworkers at were assembling concrete form machinery in preparation for manufacturing 8 ft long, 18 and 24 in. diameter concrete storm drain pipes. The crew was placing a steel adaptor/header ring horizontally into a Neptune pipe fabricating machine. An accident occurred and Employee #1 was struck on the head by a falling object and killed.
